Tenant want to give up contract before fixed term

Due to health issues, tenant has expressed that they wish to give up contract before the fixed term ends. Due to the issues, I suspect there will be issues paying the rent so would prefer they go early. Do I just need to issue a deed of surrender or is there anything else required?
In arrears with rent and it will probably be more than deposit - what is the best way of preparing a claim for small claims court>

Get them to surrender the tenancy and release deposit to you.

Is it worth going to small claims if they don't have any money? You can't get blood out of a stone. Sometimes it is just better to cut the loss and re-let.
