    I have served the S21 on my T and it expires in approx 4 weeks and no sign of T moving out so am starting on my court forms for a order of possession. I served the notice in person with my partner with me so assume I will need to get her to fill in a witness statement? Would the form she fills in be Form N215 - Certificate of Service of Notice?

    correct. She should exhibit a copy of what she actually served as well.
