    If a annex to a house is let, rent including all the electric does ist still need an EPC?

    Yes, apparently it does. Except that in such a case, the proprietor, rather than the tenant, may want to take a particular interest in how energy efficient (or not) the property is, as s/he is the one paying the energy bills.
